In this role, she expertly coordinates patient care by managing medication administration and liaising with vendors who visit patients’ homes. Her responsibilities include providing guidance on patient needs such as blood drawing, managing patient calls, answering questions and conducting assessments via Zoom. Her expertise in bringing patients home and setting them up with technology for remote assessments has been instrumental in her success.
Before joining Oregon Health and Science University, Ms. Chand was a registered nurse at Adventist Health from 2016 to 2022. During her tenure, she played a pivotal role in the hospital-at-home program until its closure, after which she was referred to her current position. Prior to this, she served as the manager for home health and hospice at Adventist Health, showcasing her leadership skills and commitment to patient care.
Ms. Chand’s career began as a Certified Nurse assistant in 1998. She worked at various bay area nursing homes through a registry. Before venturing onto her professional path, Ms. Chand marked her academic journey with significant achievements that propelled her career forward. After pursuing pre-med coursework at Foothill College, she earned a certificate in vocational nursing (LVN) from NCP Vocational School in 2001 and worked at the Palo Alto Medical Foundation from 2001 to 2010, where she gained valuable experience in clinical settings. Her early endeavors also included positions at Joe Healy Detox, as an LVN.
Ms. Chand earned an associate degree in nursing from Unitek College in 2011. She obtained a Bachelor of Science in Nursing with a focus on management and leadership in 2022 and went on to earn a Master of Science in Nursing with the same focus from Western Governors University in 2023. Her dedication to the nursing profession is further evidenced by her membership in the American Nursing Association.
Beyond her professional life, Ms. Chand has been actively involved in civic-minded organizations since her early years. She began as a candy striper with the Sequoia Hospital Foundation, a nonprofit organization dedicated to raising support and funds for various patient care programs, and has contributed to community welfare through volunteer work at soup kitchens and food pantries.
Ms. Chand attributes her success to several important figures in her life. She is especially grateful to her late grandmother, Deo Kumari, her mother, Anjana Lachan, her sister, Astika Nayyar, and her children, Amaya and Maleah, all of whom inspire her to be an ideal role model and to give her best every day. Additionally, she appreciates the valuable guidance of her mentor, Sharon Kuffel, a registered nurse who played a crucial role in the early stages of her professional career, as well as Nancy Runyan, another registered nurse who was instrumental in helping her gain leadership and management skills. In her personal life, Ms. Chand enjoys walking and hiking on nature trails as part of her leisure activities. Looking to the future, she aims to earn a Doctor of Nursing Practice and dreams of owning a boarding care home for the elderly, reflecting her enduring commitment to health care and community service.
